A new Harvard University study suggests that commercial aviation professionals confront substantially elevated risks of developing radiation-induced cancers compared with the general working population. Researchers analyzing nearly 13 million U.S. death certificates from 2020 through 2024 identified pilots and flight attendants as occupational groups with the highest mortality rates linked to radiation-related malignancies among more than 500 professions examined.
The investigation, published August 17 in JAMA Internal Medicine, found that 6.9 percent of flight attendant deaths and 6.7 percent of pilot deaths involved radiation-related cancers. This rate substantially exceeded figures for all other occupational categories studied, according to lead researcher Anupam B. Jena, M.D., Ph.D. The research encompassed 14,000 pilot and 7,000 flight attendant death records within the sample.
Exposure to cosmic radiation at high altitudes appears to be the primary culprit behind the disparity. Airline personnel receive approximately 3 to 6 millisieverts of cosmic radiation annually—roughly 8 to 15 times the exposure level of someone taking 10 cross-country round trips yearly, the Centers for Disease Control and Prevention notes. This prolonged exposure accumulates significantly over decades-long aviation careers.
Pilots and flight attendants demonstrated statistically elevated mortality from breast cancer, central nervous system cancers, prostate cancers, and melanoma. Pilots additionally showed heightened leukemia mortality rates. For non-radiation-related cancers, aviation professionals displayed rates comparable to other occupational groups, suggesting cosmic radiation rather than lifestyle factors drove the elevated mortality.
Dr. Jena indicated that enhanced cancer screening protocols may warrant consideration for aviation workers. Earlier and more frequent screenings could potentially improve outcomes, though approaches vary internationally, he noted. The findings should not substantially concern frequent business or leisure fliers, as few accumulate flight hours approaching those of career aviation professionals.
The study possesses notable limitations, as its observational design cannot definitively establish causation between cosmic radiation and cancer mortality, only association. Researchers lacked access to individual radiation exposure data, preventing determination of dose-response relationships among exposed workers. Other occupational or lifestyle factors may also contribute to the observed patterns.
