A groundbreaking clinical trial has demonstrated that a custom-designed mRNA vaccine developed by Merck and Moderna substantially lowers the likelihood of melanoma returning or spreading in patients with advanced skin cancer. The phase 3 study, called INTerpath-001, marks the first successful late-stage trial of an mRNA-based individualized cancer treatment, the pharmaceutical companies announced.
The trial enrolled 1,137 patients worldwide who had undergone surgery to remove high-risk melanoma at stages IIB through IV. Participants were randomly divided into two groups: one receiving KEYTRUDA combined with the personalized vaccine, and another receiving KEYTRUDA with a placebo. Those given the combination therapy showed substantially improved recurrence-free survival rates and distant metastasis-free survival, meaning their disease was less likely to return or spread to other parts of the body.
Unlike conventional vaccines that protect against external pathogens, the therapeutic vaccine known as intismeran autogene operates differently. After surgical tumor removal, doctors extract genetic material from both the cancerous tissue and healthy cells, then use sophisticated algorithms to identify up to 34 tumor-specific neoantigens. The vaccine is then tailored to train the immune system to recognize and eliminate remaining cancer cells unique to that individual patient.
Dr. Murad Alam, chief of dermatology at Northwestern Medicine in Chicago, emphasized that the individualized approach addresses a fundamental challenge in cancer treatment. “Every melanoma is slightly different in every patient, which is why traditional chemotherapies often stop working,” Alam told Fox News Digital. The targeted approach allows the immune system to focus on cancer-specific markers without damaging healthy cells.
Artificial intelligence played a crucial role in developing the vaccine for each patient. Alam noted that determining which antigens to include would be infeasible without AI’s computational power, though he expects the technology to improve further. The personalized nature of the treatment means side effects remain minimal, typically limited to mild fatigue, chills, and injection site pain.
The vaccine is designed specifically for patients with advanced melanoma that threatens their overall health, not for prevention in healthy individuals or those with family risk factors. Alam stressed the vaccine targets only advanced cases where the disease has progressed significantly beyond localized skin involvement.
Merck and Moderna plan to pursue regulatory approval from the Food and Drug Administration and other worldwide authorities based on these findings. The companies are also exploring whether the vaccine technology can be adapted for other difficult-to-treat cancers, including lung, bladder, and kidney cancers.
While the results are encouraging, researchers cautioned that the trial did not establish whether the vaccine ultimately extends patient survival or how long patients remained cancer-free. The recurrence-free survival data relied on investigator assessments rather than independent central review, and the study was limited to specific melanoma stages in patients without prior drug treatments, meaning findings may not apply to other cancer types or stages.
Safety data showed no new concerns, with adverse effects consistent with earlier studies. Alam highlighted that the vaccine’s targeted approach distinguishes it from conventional cancer medications, which often cause severe side effects and significant suffering during treatment courses.
